These Terms apply to all patients, website visitors, and any other individuals who interact with SmileCare Dental Clinic in any capacity. For patients under 18 years of age, a parent or legal guardian must agree to these Terms on their behalf.
SmileCare Dental Clinic provides a range of professional dental services including, but not limited to, general dentistry, cosmetic dentistry, orthodontics, oral surgery, and preventive care. All services are provided by licensed dental professionals in accordance with applicable Texas state regulations and professional standards.
The dental services described on our website and marketing materials are for informational purposes only. Treatment recommendations are made on an individual basis following a professional examination and diagnosis. SmileCare Dental Clinic reserves the right to decline service to any patient if, in the professional judgment of our dental staff, the service cannot be safely or ethically provided.

Appointments may be scheduled online, by telephone, or in person. All appointment requests submitted through our website are subject to confirmation by our scheduling team. Receipt of an online form submission does not constitute a confirmed appointment until you receive a confirmation call or email from our office.
Patients are expected to arrive at least 10 minutes before their scheduled appointment time to complete any required paperwork and check-in procedures. Late arrivals may result in a shortened appointment or rescheduling, at the discretion of our clinical staff.
SmileCare Dental Clinic reserves the right to reschedule or cancel appointments due to unforeseen circumstances, including but not limited to provider illness, equipment failure, or emergency situations. We will make every reasonable effort to notify you in advance and offer an alternative appointment time.
For new patients, a comprehensive oral examination may be required before specific treatments can be scheduled. Emergency dental appointments are available during regular business hours and, in certain circumstances, by after-hours arrangement.
We understand that circumstances change and appointments sometimes need to be rescheduled. We ask for a minimum of 48 hours' notice for appointment cancellations or changes. This allows us to offer the appointment slot to other patients who may need care.
Cancellations made with less than 24 hours' notice, or failure to appear for a scheduled appointment without prior notice, may result in a late cancellation or no-show fee. This fee is $50 for standard appointments and $100 for specialty or surgical appointments.
Patients with two or more no-show appointments within a 12-month period may be required to prepay for future appointments or may be discharged from the practice, at SmileCare's discretion.

Payment for dental services is due at the time services are rendered, unless a prior payment arrangement has been established with our billing department. We accept cash, personal checks, Visa, Mastercard, American Express, Discover, and CareCredit financing.
For patients with dental insurance, we will submit claims on your behalf as a courtesy. However, you remain ultimately responsible for payment of all services rendered. Any amounts not covered by your insurance, including deductibles, co-payments, co-insurance, and non-covered services, are your responsibility and are due at the time of service.
Unpaid balances are subject to a monthly finance charge. Accounts more than 90 days past due may be referred to a collection agency, and collection costs, including reasonable attorney fees, may be added to the outstanding balance.
Treatment plans and cost estimates provided by SmileCare Dental Clinic are estimates only and may change based on the actual services required during your treatment. Final charges are based on services actually performed.
SmileCare Dental Clinic accepts most major dental insurance plans. It is your responsibility to verify your insurance coverage before your appointment. We recommend contacting your insurance provider directly to confirm coverage, deductibles, co-payment amounts, and any pre-authorization requirements.
We will make every reasonable effort to verify your insurance benefits before your appointment. However, insurance verification does not guarantee payment by your insurer. Insurance companies have the right to make final coverage determinations after reviewing the claim.
If your insurance company denies a claim or pays less than anticipated, you are responsible for the balance. Misrepresentation of insurance information may result in immediate termination of the patient-provider relationship and potential legal action.

SmileCare Dental Clinic reserves the right to discontinue care for any patient who engages in abusive, threatening, or disruptive behavior. In such cases, we will provide a reasonable period of continued care for emergency needs and assist with referrals to other providers.
SmileCare Dental Clinic maintains comprehensive dental records for all patients. These records are the property of the clinic but patients have the right to access copies of their records in accordance with applicable law. Requests for records should be submitted in writing and may be subject to a reasonable copying fee.
